This session will explore how General Practice teams can support women to understand and manage their future risk. It will also highlight practical resources and approaches to improve care.
Kate Leadley, Advanced Nurse Practitioner and Diabetes Champion for Hillsborough PCN, and Clair Thompson, Primary Care Diabetes Nurse (PCDN), will share learning and outcomes from a successful General Practice improvement project focused on women with a history of GDM.
